Bone density refers to the amount of mineral content in bone. It helps indicate how strong or fragile bones may be and is one factor used to assess fracture risk.


Bones can feel normal even when bone density has decreased. This is why bone density testing may be considered when there are risk factors for bone loss, fractures, or hormone-related conditions that may affect bone health.

What Bone Density Means


Bone density measures how much mineral, mainly calcium and phosphate, is present in bone.

  • A single result does not show the full picture of bone health


Bone strength is influenced by bone density, bone structure, age, hormones, nutrition, activity levels, and overall health.


How Bone Density Is Measured


The most common test used to measure bone density is a DEXA scan, also called dual-energy X-ray absorptiometry.


A DEXA scan usually measures bone density at:

  • The hip

  • The lumbar spine

  • Sometimes the forearm, depending on clinical need


Results are often reported using a T-score, which compares a person’s bone density with that of a young healthy adult.


A doctor may also consider other factors, such as age, fracture history, medication use, and medical conditions.


Osteopenia and Osteoporosis


Bone density results may help identify osteopenia or osteoporosis.


Osteopenia 

This means bone density is lower than average but does not meet the threshold for osteoporosis.


Osteoporosis 

This means bone density is reduced to a level associated with increased bone fragility and fracture risk.


Fractures related to low bone density may occur in areas such as:

  • Hip

  • Spine

  • Wrist


These conditions should be assessed by a healthcare professional and interpreted in the context of the person’s overall health.


Why Bone Density May Decline


Bone density usually reaches its peak in early adulthood and may gradually decline with age. Some people may lose bone density more quickly due to hormonal, medical, lifestyle, or medication-related factors.


Possible contributing factors include:

  • Menopause-related oestrogen decline

  • Low testosterone levels

  • Overactive parathyroid glands

  • Overactive thyroid function

  • Long-term corticosteroid use

  • Low calcium or vitamin D levels

  • Low body weight

  • Smoking

  • High alcohol intake

  • Reduced weight-bearing activity

  • Family history of osteoporosis or fractures


These factors do not confirm low bone density, but they may guide whether assessment is appropriate.


How Bone Density Results Are Used


A bone density result is not usually used on its own. It is reviewed together with clinical information to better understand fracture risk.

A medical review may consider:

  • Age and sex

  • Previous fractures

  • Family history

  • Falls risk

  • Calcium and vitamin D status

  • Medication use

  • Medical conditions

  • Hormonal factors

  • Lifestyle and activity levels


Some people with low bone density may need monitoring and lifestyle review. Others may require further evaluation or treatment discussion, depending on their risk profile and clinical findings.


Hormones and Bone Density


Hormones can affect how bone is maintained and renewed over time.


Hormonal conditions that may influence bone density include:

  • Hyperparathyroidism

  • Hyperthyroidism

  • Low oestrogen levels

  • Low testosterone levels

  • Cushing’s syndrome or long-term cortisol excess

  • Conditions affecting calcium or vitamin D balance


When hormonal factors are suspected, further assessment may help clarify whether they are contributing to bone loss.


When to Discuss Bone Density With a Doctor


You may consider speaking with a doctor about bone density if you have:

  • A history of fracture from a minor fall or injury

  • A family history of osteoporosis or hip fracture

  • Menopause-related bone health concerns

  • Long-term corticosteroid use

  • Thyroid, parathyroid, or reproductive hormone conditions

  • Low calcium or vitamin D levels

  • Low body weight

  • Concerns about falls or fracture risk
